Lutetium-177 PSMA therapy as the first-line treatment for metastatic prostate carcinoma.

A 77-year-old gentleman presented with metastatic prostate carcinoma and significant pain — left leg weakness, left hip pain, and muscle wasting. With no prior treatment, the team initiated upfront Lu-177 PSMA-617 therapy. After two cycles, his PSA dropped from >2,000 ng/ml to 10.2 ng/ml with significant pain relief and improved quality of life.

Lutetium-177 PRRT for metastatic well-differentiated neuroendocrine tumour.

A 35-year-old gentleman with a metastatic well-differentiated NET had been on monthly Sandostatin-LAR 30 for six months without symptom relief and with progressive disease. After 3 cycles of Lu-177 PRRT at 3-month intervals, his Chromogranin A levels dropped from 353 to 132 with a 42% reduction in tumour volume on Ga-68 DOTANOC PET-CT, plus complete relief of symptoms.
