For online consultations, the ₹ 2,500 fee is billed only once a slot is confirmed; OPD appointments carry no TPPL fee, only FMRI's standard hospital charges.
Eligibility: the person submitting this form must be 18 years of age or older, and the patient must be an Indian national or a foreign national physically resident in India at the time of the consultation. The service is not available to persons located outside India.

For an OPD appointment, TPPL acts as a scheduling conduit — connecting you to the consultant's office at Fortis Memorial Research Institute. The clinical consultation, billing, and any treatment that follows are governed by FMRI's own protocols and fee schedule, not TPPL's. There is no online consultation fee on this path; FMRI's OPD charges apply at the hospital.
Documents to bring on the day (recommended, not mandatory): the most recent scan report, prior scan reports if available, pathology, recent biochemistry, prior treatment notes, and a government-issued identity card. The administrator will confirm the appointment date and time with you over email or WhatsApp once your request is reviewed.
